Summary : The cerebrospinal fluid (CSF) is an invaluable diagnostic tool in clinical neurology, not only in the evaluation of inflammatory, degenerative, and malignant diseases of the nervous system, but also in the diagnosis of all forms of cerebral and subarachnoidal bleedings. The CSF can be easily obtained by lumbar puncture and a set of basic analyses can be conducted using relatively simple laboratory methods. By combining different CSF parameters, a wide range of diagnostic entities can be identified. However, properly interpreting the test results requires a high level of expertise and cannot be achieved by just reporting on individual analytic values. This book covers essential aspects of cerebrospinal fluid analysis and its use in the diagnosis of common neurological diseases. The first part addresses preclinical aspects such as the history of CSF, as well as the anatomical, physiological, and biological background of this valuable fluid. In addition, CSF collection, its preanalytical and methodological implications, and the increasing number of disease-specific markers in CSF are discussed in detail. Lastly, CSF analyses are put into context with clinical syndromes, demonstrating their diagnostic value in neurological clinical practice. Cerebrospinal Fluid in Clinical Neurology helps readers understand the preanalytical and analytical aspects of CSF diagnostics and offers a valuable reference guide for interpreting CSF results during the clinical work-up for neurological patients.

Summary : This updated edition provides clinicians from various backgrounds and levels of training the information needed to optimally diagnose and manage neurologic complications of the nervous system. Organized into seven sections, this comprehensive volume begins with an overview of diagnostic studies for neurologic complications involving the nervous system. That is followed by sections on metastatic and non-metastatic complications of cancer involving the nervous system, and the interpretation, diagnosis, and management of common neuro-oncologic symptoms. The next section reviews the neurologic complications of cancer therapy, including corticosteroids, radiation therapy, chemotherapy, targeted molecular therapies, immunotherapies, hematopoietic stem cell transplantation, and infections involving the nervous system. The final section focuses on the most important neurologic complications in cancers arising from specific organs. In addition to capturing the latest advancements in the rapidly evolving fields of oncology and cancer neurology, the goal of this resource is to lead clinicians toward prompt diagnosis and intervention in order to improve patient quality of life. "This textbook is a valuable resource for medical oncologists and radiation oncologists, as well as neurologists and neuro-oncologists dealing with these patients. ... Overall, the chapters are well organized, clearly written, fairly balanced, and reasonably up to date. ... I would recommend it as a learning tool to physicians in training (medical students, residents, and fellows) and for more experienced physicians as both a review/ update and a way to gain more in-depth knowledge and insight into the neurologic problems of cancer patients." (John C. Flickinger, International Journal of Radiation Oncology Biology Physics, Vol. 73 (2), 2009) "The general organization of the book is logical and facilitates its practical and everyday use. ... Overall this textbook is very comprehensive and encompasses main neuro-oncological challenges. ... Schiff, Kesari and Wen have edited a very elegant and highly practical textbook, written by recognized authorities in their respective fields, which will be used by a wide range of medical and surgical specialists who are confronted on a daily basis with neurological manifestations of cancer in their practice." (I. Radovanovic and G. Zadeh, British Journal of Cancer, Vol. 100 (6), 2009)

Summary : Traumatic brain injury (TBI) is a major cause of death and disability and one of the greatest unmet needs in medicine and public health. TBI not only has devastating effects on patients and their relatives but results in huge direct and indirect costs to society. Although guidelines for the management of patients have been developed and more than 200 clinical trials have been conducted, they have resulted in few improvements in clinical outcomes and no effective therapies approved for TBI. It is now apparent that the heterogeneity of clinical TBI is underlain by molecular phenotypes more complex and interactive than initially conceived and current approaches to the characterization, management and outcome prediction of TBI are antiquated, unidimensional and inadequate to capture the interindividual pathophysiological heterogeneity. Recent advances in proteomics and biomarker development provide unparalleled opportunities for unraveling substantial injury-specific and patient-specific variability and refining disease characterization. The identification of novel, sensitive, objective tools, referred to as biomarkers, can revolutionize pathophysiological insights, enable targeted therapies and personalized approaches to clinical management. In this Research Topic, we present novel approaches that provide an infrastructure for discovery and validation of new biomarkers of acute brain injury. These techniques include refined mass spectrometry technology and high throughput immunoblot techniques. Output from these approaches can identify potential candidate biomarkers employing systems biology and data mining methods. Finally, suggestions are provided for the way forward, with an emphasis on need for a multidimensional approach that integrate a panel of pathobiologically diverse biomarkers with clinical variables and imaging-based assessments to improve diagnosis and classification of TBI and to develop best clinical practice guidelines.

Summary : Most neurological disorders are chronic and aging-related. With the increase of life expectancy their incidence and prevalence will grow in the decades to come, which in turn will increase the load on medical and social systems worldwide. There is thus a desperate need for successful preventive and therapeutic measures based on randomized clinical trials (RTCs) conducted by independent organizations. This book provides a compendium relating most of the principles of reliable RTCs to specific neurological diseases. Contributed by specialized neurologists, the articles touch on important aspects of RCTs with a clear critical approach, highlighting their limitations as well as giving recommendations for their planning and conducting to address the variable genotypic and phenotypic aspects of neurological conditions. Consideration is also given to combining the clinical impact of the study results with patients’ values and the interests of pharmaceutical companies. Neurologists involved in clinical trials will certainly benefit from this book, which should become a basic text for all neurological courses dealing with evidence-based neurology.
