Indeed, it is becoming clear that, as a molecular entity, a glioblastoma, which by traditional classification is a grade IV glioma, may present with a lower grade initially and then become more aggressive – an important addition to the established concept. The care of a patient with a glioblastoma requires a coordinated approach delivered by a multidisciplinary team, with the aim of maintaining quality of life for as long as possible. Here, we provide a concise overview of the diagnosis and management of glioblastoma, as well as discussion of our emerging understanding of the molecular drivers that are helping us to delineate different patient subgroups. These subgroups will, hopefully, allow more targeted treatments in the future. Summary : Abstract : Accomplishing significantly enhanced overall survivability (OS) remains a formidable challenge in combating glioblastoma. The presence of the blood–brain barrier acts as the major biological barrier in delivering drugs to the brain. Herein, liposomal formulations of two novel nicotinylated amphiphiles are reported for targeting potent anticancer drugs to orthotopic mouse glioblastoma. It is shown that intravenous administration of the potent signal transducer and activator of transcription 3 (STAT3) inhibitor (WP‐1066)‐loaded liposomes of nicotinylated amphiphiles in combination with in vivo dendritic cell (DC)‐targeted subcutaneous genetic immunization (using tyrosinase‐related protein‐2 encoded DNA vaccine) markedly enhances the OS of orthotopic glioblastoma‐bearing mice (by >500% compared to the OS for the control group). Notably, the overall survival benefits in orthotopic‐brain‐tumor‐bearing mice treated with only targeted chemotherapy or with only in vivo DC‐targeted genetic immunization are found to be significantly less. The presently described simple approach avoids the need of isolation of any autologous immune cells. In summary, the preclinical findings described herein open the door for combating glioblastoma in humans through harnessing synergistic effects of targeted chemotherapy and in vivo DC‐targeted genetic immunization. Abstract : The present study reports on combating established orthotopic mouse glioblastoma by targeted chemotherapy in combination with in vivo dendritic cell (DC)‐targeted genetic immunization. The combination approach elicits enhanced overall survivability (more than 500% compared to untreated mice) of orthotopic glioblastoma bearing mice without the need of isolation and ex vivo engineering of recipient's autologous immune cells.

In this new millennium, it is now possible to envision a much more diversified system of cancer research and treatment that will afford greater opportunities for a patient’s personalized cancer treatment. This was first envisioned in the 1987 initial edition of this textbook and is now a “new” and popular approach to cancer treatment. Some forms of cancer biotherapy use the strategy of tumor stabilization and control through continued biological therapy, akin to the use of insulin in the treatment of diabetes. This textbook illustrates new methods of thinking and new strategies for control of cancer. It is always difficult to move from past dogma to future opportunity, but this fifth edition of Principles of Cancer Biotherapy illustrates why it is so important to the patients for researchers and clinicians to explore and quickly apply these new opportunities in cancer biotherapy.
