Summary : Video game addiction is the latest behavioral process disorder. While it is similar to substance misuse disorders and problematic gambling, little is known about pathology and almost nothing about treatment. It was added to the latest version of the International Classification of Diseases (ICD-11), which resulted in headline news around the world in June of 2018. It is expected that it will be added the next version of the DSM (DSM 5.1), the mental health diagnostic book that is used in the United States. To put the gaming industry in perspective, consider that worldwide revenue for McDonalds was $22 billion in 2017. U.S. only revenue from video games in 2018 is projected to be $30 billion. Furthermore, very few non-gamers know about loot boxes. These are boxes/chests in games that one can spend money on in order to acquire desired items. Loot boxes are cheaper than most desired items in the box, so players are hoping that it contains what they want (it usually doesn't). The authors view loot boxes as gambling, as do some European governments. We believe that loot boxes are priming a few generations for gambling problems. This book examines the history of gaming and then devotes individual chapters to gaming in the United States and several Asian countries. Since video games emerged as a public health crisis in South Korean, Japan, China and Thailand about a decade earlier than the United States, there is much we can learn from their research and actions. Summary : Introduction: Our development and clinical use of a novel self-report questionnaire in the Los Angeles San Fernando Child and Family Mental Health Clinic, The Video Game Initial Questionnaire (VGIQ), shows that a key element of impairment related to excessive internet gaming and on-line activities in youth is sleep disruption (Pataki et al., 2019). Sleep can be compromised due to post-bedtime on-line usage, particularly when parents are asleep (Pataki et al., 2019). As a result of chronic fatigue in youth, many daytime activities can be negatively affected. In fact, every clinic patient who completed the VGIQ reported that video game use interfered with homework, relationships, school achievement and/or sleep. A new disorder u201c Internet Gaming Disorderu201d (Gentile et al., 2016, Paulus et al., 2018, ICD 11, 2018), has been introduced characterized by a preoccupation with games, deceiving others as to how much time is spent playing, irritability or sadness when gaming is restricted, compromised relationships and continued excessive game use despite awareness of impairment. Given the many negative impacts of internet over-use, our clinicians now include a careful review of sleep habits, and then provide parents with interventions to monitor and decrease night-time game playing and thereby improve childrenu2019s sleep. The gap between youth and parental knowledge of technology was noted to be a risk factor for excessive night-time gaming. Sleep disruption may be an early marker of problem behavior where intervention can make a critical first step in improvement of childu2019s overall functioning. Objectives: 1) To present ways to help parents identify their childrenu2019s actual video game play time, after bedtime play and hours of sleep.2) To provide parents with a better awareness of 24-hour availability of electronic entertainment so they can better monitor and limit gaming behaviors. 3) To provide clinicians with information to become more effective with families to understand, monitor, and decrease gaming play time in youth, in order to prevent Internet Gaming Disorder and protect daytime overall functioning Method : Clinic patients presenting with sleep disturbance related to excessive gaming meet with clinicians individually and together with their parents in order to elicit sleep habits and hygiene, total number of hours the child is playing games and hours spent playing without parental knowledge when parents are asleep. Behavioral strategies are presented for parents to implement in order to decrease the number of hours their child is game- playing, and to increase sleep time. Improved understanding of the u201celectronic divideu201d between parents and children as well as improved clinician awareness is discussed.CONCLUSIONS: 1) Parents can successfully work with their children and clinicians in therapeutic sessions to elicit unknown information regarding their childu2019s video game usage and habits and their disturbances due to nighttime playing. 2) Sleep disruption associated with excessive gaming in youth at night may be an initial marker of future developing impairment in school, family and peer relationships.3) When sleep disruption has come to light, early intervention by parents, patients and clinicians to implement effective and practical tools to limit late night gaming and improve childrenu2019s sleep may lead to protection and improvement of overall function in a new and changing electronic world.
