Summary : "Why are there no effective treatments for my condition? Why do researchers exclude patients with primary progressive multiple sclerosis from enrolling in clinical trials? Please let me know if you hear of studies that I might be allowed to enter or treatments that I could try for my condition. " Thus, in recent years, the sad lament of the patient with primary progressive MS (PPMS). This variant, often in the guise of a chronic progressive myelopathy or, less commonly, progressive cerebellar or bulbar dysfunction, usually responds poorly to corticosteroids and rarely seems to benefit to a significant degree from intensive immunosuppressive treatments. In recent years, most randomized clin ical trials have excluded PPMS patients on two counts. Clinical worsening devel ops slowly in PPMS and may not be recognized during the course of a 2-or 3-year trial even in untreated control patients. This factor alone adds to the potential for a type 2 error or, at the very least, inflates the sample size and duration of the trial. In addition, there is mounting evidence that progressive axonal degeneration and neuronal loss (rather than active, recurrent inflammation) may be important components of the pathology in this form of the disease. Although contemporary trials are evaluating whether PPMS patients may benefit from treatment with the ~-interferons and glatiramer acetate, preliminary, uncontrolled clinical experi ence suggests that the results may not be dramatic.

Summary : Multiple sclerosis is a chronic and often disabling disease of the nervous system, affecting about 1 million people worldwide. Even though it has been known for over a hundred years, no cause or cure has yet been discovered-but now there is hope. New therapies have been shown to slow the disease progress in some patients, and the pace of discoveries about the cellular machinery of the brain and spinal cord has accelerated. This book presents a comprehensive overview of multiple sclerosis today, as researchers seek to understand its processes, develop therapies that will slow or halt the disease and perhaps repair damage, offer relief for specific symptoms, and improve the abilities of MS patients to function in their daily lives. The panel reviews existing knowledge and identifies key research questions, focusing on: Research strategies that have the greatest potential to understand the bio- logical mechanisms of recovery and to translate findings into specific strategies for therapy. How people adapt to MS and the research needed to improve the lives of people with MS. Management of disease symptoms (cognitive impairment, depression, spasticity, vision problems, and others). The committee also discusses ways to build and financially support the MS research enterprise, including a look at challenges inherent in designing clinical trials. This book will be important to MS researchers, research funders, health care advocates for MS research and treatment, and interested patients and their families.

Summary : Although a strikingly modem account of multiple (disseminated) sclerosis was given by Charcot more than one hundred years ago, we are still not sure of the precise nature of the disease, still less of any precise mode of management. Even diagnosis is at best 'probable'. This collection of essays examines the particular difficulties which beset the problem and have mili tated against the solution-problems of data collection and evaluation; clinical and biochemical vagaries and unanswered questions ; laboratory test prediction of the disease, etc. Experts in their fields have set out the difficulties and way forward as ther see them, and there must necessarily be overlap or contradictions which underline our lack of knowledge. Clearly no attempt can be made to cover more than a very few aspects of the problem (a further volume is planned to review other areas not covered here, in particular the relationship between multiple sclerosis and experimental allergic encephalo myelitis)-but it is hoped that the 'softness' of much of the data we have to work upon will become appan!~nt. It is intended that this volume should clearly differ from the large number of monographs and reports of symposia dedicated to multiple sclerosis in bringing out the shortcomings of our approaches, and in some respects our naivities rather than our individual brilliancies.

Summary : Multiple Sclerosis (MS) is an enigmatic immune mediated disease of the central nervous system that affects about 350,000 individuals in the US, and many more around the world. The mechanism of this disease is largely unknown and there is no cure for it. However, there are several well-characterized experimental animal models that help us understand and speculate about potential mechanisms of pathology in this disease. Many of the experimental therapies designed for this disease rely on testing the drugs in animal models before using it in clinical trials. This book combines for the first time the different experimental models for MS (including immune-mediated and viral) under one roof, and highlights aspects that are different or shared among these experimental models. It’s aim is to improve our understanding of this devastating disease and help us think about potential additional therapies for it.

Summary : Progressive multiple sclerosis places a huge burden on individuals, carers and society and treatment options are currently limited. However, there have been significant recent advances in understanding progressive disease and this book draws the field together into a single, accessible, clinically focused text. Progressive Multiple Sclerosis sets out the basic science, pathology, epidemiology and treatment possibilities in progressive multiple sclerosis. The biological processes underlying disease progression are elucidated as are specific pathophysiological mechanisms. The basic science of axonal degeneration and the clinical understanding of progressive multiple sclerosis are discussed in the context of emerging and future therapies for disease progression as are trials which might be conducted for progressive multiple sclerosis. An overview of biomarkers for the disease is included and existing symptomatic therapies for progressive multiple sclerosis are covered in detail. Each chapter addresses specific aspects of progressive multiple sclerosis providing an extensive review of the current literature and a discussion of the likely mechanisms of tissue injury relevant to the disease. Progressive Multiple Sclerosis is ideal for both general neurologists and those with a specialist interest in multiple sclerosis. It will also be of interest to neurologists in training and other medical professionals treating this common disease. Dr Alastair Wilkins is Senior Lecturer in Neurology, University of Bristol, and Consultant Neurologist at Frenchay Hospital, Bristol, UK. He researches axonal injury in multiple sclerosis and related disorders.

Summary : Multiple Sclerosis (MS) is generally understood to be an inflammatory autoimmune disease of the central nervous system. While we still are not certain of the root cause of MS, research results suggest that unknown environmental factors and the presence of specific genes seem the most probable targets. MS causes an inflammatory response in the central nervous system leading to neurodegeneration, oligodendrocyte death, axonal damage, and gliosis. Over the past five years ongoing research has greatly expanded our understanding of the pathogenesis of MS, detailed insight into the epidemiology and genetic factors related to MS, the introduction of new technologies and tests to better diagnose and predict the future course of the disease and the introduction of new treatments targeting MS. This collection of review chapters provides a comprehensive reference into the science and clinical applications of the latest Multiple Sclerosis research and will be a valuable resource for the neuroscience research community and the clinical neurology community of researchers and practitioners. A comprehensive tutorial reference detailing our current foundational understanding of Multiple Sclerosis Includes chapters on key topics including the genetics of MS, MRI imaging and MS, and the latest treatment options Each chapter is translational and focuses on current research and impact on diagnosis and treatment options

Summary : Multiple sclerosis (MS) is a chronic inflammatory disease characterized by progressive demyelination and neurodegeneration of the central nervous system (CNS), constituting the most common demyelinating disease of the CNS in humans. Although intensive research over many decades has unveiled many pathophysiological mechanisms in the development of MS, the cause is still unknown. Nevertheless, it does seem clear that genetic susceptibility and environmental factors play crucial roles. Trending Topics in Multiple Sclerosis is a book that provides an insight into some of the main problems currently debated in this area of research, focusing on topics that deal with genetic and environmental risk factors, pathophysiological mechanisms, neurocognitive findings, and neuroprotective strategies.
