Summary : Pathological conditions affecting the hip and knee joints occupy a particular place amongst the important orthopaedic entities affect ing the extremities. On the one hand they are relatively frequent and on the other they mean for the patient limitation of his ability to walk, because of their considerable detrimental effects. A purposeful basic treatment of these joint diseases (and here osteoarthritis takes pride of place) is only possible if it stems from a reliable biomechanical analysis of the normal and pathological stressing of the joint in question. Whilst the situation in the hip can be considered to be fundamentally clarified, a comprehensive representation of the knee is still lacking, particularly when taking into account the latest knowledge of biomechanics. Recently our concepts of the kinematics of the knee have been completely changed, but the clinically important question of articular stressing remains unanswered. Dr. Maquet has carried out pioneer work in this field for some years in adapting, by analogy, to the knee joint principles already accepted for the hip joint. Since the knee is not a ball and socket joint, a complicated problem arises for which new thoughts are necessary. The results of the numerous operations carried out by Dr. Maquet according to the biomechanical considerations demon strate that his thinking is fundamentally correct. Above all, it is here again proven (as earlier in the case of the hip) that healing of osteoarthritis depends decisively on reducing and evenly dis tributing joint pressure.

Summary : This book represents the most advanced understanding of diagnosis and management of hip dysplasia in the young adult, written by the world’s leading experts and covering advanced imaging and biomechanical studies as well as latest surgical techniques. This is especially timely, as the field of hip preservation surgery is in constant evolution, and it is critical that surgeons and researchers combine their efforts to provide the best evidence-based knowledge to all health care providers evaluating and treating patients suffering from pre-arthritic hip pain. Not only does this book discuss the most up-to-date information on the etiology and natural history of hip dysplasia, as well as pathomechanics and traditional and advanced imaging, it also presents new conceptions of its classification and subsequent management. The focus throughout the book is that hip dysplasia is a problem of instability, where both the bony structure and the soft tissue envelope (i.e., capsule, ligaments and muscles) are at play. Moreover, although corrective osteotomies such as the peri-acetabular osteotomy will remain the dominant treatment option, non-surgical treatments as well as hip arthroscopy are now being increasingly recognized as useful adjuncts in order for our patients to return to their desired activities. Finally, as the longevity of joint replacement surgery has improved dramatically, this should be strongly considered in cases where the outcome of joint preserving surgery is not as predictable. Orthopedic surgeons, sports medicine physicians, physiatrists and physiotherapists alike will find this book a valuable resource when treating the young adult with symptomatic hip dysplasia.

Summary : This quick-reference guide is the first book written specifically for the many third- and fourth-year medical students rotating on an orthopedic surgery service. Organized anatomically, it focuses on the diagnosis and management of the most common pathologic entities. Each chapter covers history, physical examination, imaging, and common diagnoses. For each diagnosis, the book sets out the typical presentation, options for non-operative and operative management, and expected outcomes. Chapters include key illustrations, quick-reference charts, tables, diagrams, and bulleted lists. Each chapter is co-authored by a senior resident or fellow and an established academic physician and is concise enough to be read in two or three hours. Students can read the text from cover to cover to gain a general foundation of knowledge that can be built upon when they begin their rotation, then use specific chapters to review a sub-specialty before starting a new rotation or seeing a patient with a sub-specialty attending. Practical and user-friendly, Orthopedic Surgery Clerkship is the ideal, on-the-spot resource for medical students and practitioners seeking fast facts on diagnosis and management. Its bullet-pointed outline format makes it a perfect quick-reference, and its content breadth covers the most commonly encountered orthopedic problems in practice.

Summary : Dr. MAQUET, the foremost disciple of Professor PAUWELS' and the orthopae dic heir to the PAUWELS' concepts of osteotomy of the hip for arthritis, has assembled in this one book the strongest and most lucid contemporary statement of the principles and practice of this very important school of hip surgery. Professor PAUWELS' contributions to the understanding of the biomechanics of the hip and to the concepts and execution of osteotomy of the hip for arthritis are outstanding and timeless. With clarity, Dr. MAQUET articulates this position and refines it further in the light of his own investiga tion. While other investigators, of course, differ on individual concepts or princi ples in this book or disagree with specific positions, assumptions, or conclu sions, it is clear to all that this book is a benchmark work. Dr. MAQUET, as Professor PAUWELS always did, illustrates his text lavishly with beautiful examples of individual cases illuminating the principles ad vanced. But in addition, he has gone further and presents long-term follow-up data, quantifying the results of these surgical precepts as experienced in his own practice. It is a work that has been long sought and is richly received. Boston, Massachusetts, 1984 WILLIAM H. HARRIS, M.D.

Summary : The theme of this work is the application of the engmeermg theory of frictional torque to total hip replacement. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.

Summary : There is a long list of diseases and traumatic events that affect hip joint, but none as persistent, as elusive and with profound consequences as developmental dysplasia. For many centuries, we have been struggling with its consequences while trying to understand the reasons how and why such a stable joint eventually becomes dysfunctional and how to prevent it. Some of the greatest achievements in operative orthopaedics have been introduced in the effort to treat developmental dysplasia of the hip. This book offers a contemporary approach to developmental dysplasia of the hip, covering various clinically relevant aspects - historical and epidemiological considerations, biomechanical analysis, conservative methods and operative treatment procedures.

Summary : For more than 100 years, congenital dislocation of the hip has been an area of concern in orthopedics. This publications on the subject are al most too numerous to count. Yet our knowledge of the basic principles of congenital hip dislocation and its management is constantly being expanded by new research. In Germanspeaking countries, Kaiser pub lished the last comprehensive textbook on congenital hip dislocation in 1958, and Schlegel followed with a comprehensive handbook in 1961. In the Angloamerican world, Coleman's monograph was pub lished in 1978, Somerville's in 1982, and Wilkinson's in 1985. In 1982 Tachdjian compiled a volume on congenital hip dislocation that con tained contributions from 44 authorities. The purpose of the present book is to provide an overview of our pre sent state of knowledge of congenital hip dislocation, covering basic principles, diagnosis, methods of closed and open treatment, and indi cations. In the process, an attempt is made to trace progress in the field from its beginnings to the present time. Many authors describe the diagnosis and treatment of congenital hip dysplasia and dislocation in terms of specific age groups. We believe it is more prudent to take an individualized approach based on arthro graphic findings and the degree of severity of pathologic changes. Less emphasis is placed here on the management of patients by age group.
